COACHING AGREEMENT
COOPERATION:
It is assumed that both parties will be in full cooperation with each other in regards to
the agreed-upon coaching objectives. Neither party will withhold important information which would
benefit the coaching process, nor will either party act in any manner which would interfere with facilitation
of the coaching goals in a timely manner.
FEES:
Coaching fees are based on the services provided. Coach and client will meet and have followup
sessions until the client feels that his or her personal and professional goals have been met. The
minimum number of sessions will be three (3). Because several sessions are needed for the coach to
really get to know the client, meeting only three times is usually insufficient to accomplish the coaching
goals. Coaching goals can usually be met with 10 to 15 sessions, depending upon the goals and type of
coaching involved.
The coaching fee will be $50 per hour. A limited number of brief e-mails are available, at no additional
cost, between sessions, as needed. The first three coaching sessions are to be paid in advance of the
first session. Beginning with the fourth session, payment can be made at the time of the service unless
the client prefers to pay for several sessions in advance for convenience purposes. The client is under
no obligation to continue coaching beyond the third session.
When a client schedules an appointment, that block of time is reserved by the coach for that client. If the
client must change or cancel an appointment, 24 hours notice is required. Monday appointments need
to be cancelled by 6:00 PM Saturday. Without such notice, a $25 fee will be charged for the first missed
appointment or late cancellation. The full $50 fee will be charged for missed appointments and late cancellations
thereafter.
PROCEDURE:
Coach and client typically continue to schedule coaching sessions until the established
goals have been achieved. Coaching can be done face-to-face or by telephone. When the coaching is
done by telephone, it is the coach’s responsibility to call the client and bear the cost of any long-distance
telephone fees (within the USA only), and it is the client’s responsibility to be available to receive the call
in a quiet environment that will maximize the time spent with the coach.
CONFIDENTIALITY:
Confidentiality and protection of intellectual property are mutually agreed to be
protected by this contract. Both parties agree that all data, information, and work completed during the
course of life coaching will remain confidential. No information or materials will be shared with outside
sources or other people regarding the work of either party, except with express written permission of
both parties.
MODIFICATIONS AND EVALUATIONS:
Either party may modify this contract at any time it becomes apparent that modification is needed. The coach agrees to advise and influence action. However, the client is always the final decision maker in the coaching process.
I affirm that the information given on this
